Health Center, Student

Health and Safety Center 114
706-568-2039

The Student Health Center offers a variety of services provided by a professional staff of nurses and physicians. These include:

  • Outpatient Clinic. A family practice physician provides general medical care on a walk-in basis and assists with referrals for specialty or hospital care as needed.
  • Allergy Clinic. Staff administer allergy injections at the direction of the student’s own physician. Both medication and instructions for administration must be provided.
  • Health Education. Health Center personnel provide information on health-related topics to individuals and groups requesting assistance.
  • Women’s Health Clinic. The Clinic provides routine examination and treatment of gynecological problems, PAP smears, contraceptive services, pregnancy testing and the diagnosis and treatment of sexually transmitted diseases. The Women’s Health Clinic is held on the second and fourth Tuesday of each month that school is in session and appointments are required.
  • Pharmacy Service. When clinic physicians prescribe medication, a limited number of discounted prescription drugs are available from the Clinic. Common over-the-counter medications, condoms, and vitamins are offered free or at low cost to students.

There is no charge for most medical services to currently enrolled students. Charges are made for laboratory services and for certain supplies. Payment for any charges is due at the time of service. Unpaid accounts will prevent a student from registering or receiving grades or transcripts.

A group student health and accident insurance is available to all CSU students. Information and application may be obtained in the Student Health Center or in Student Services, Health and Safety Center 114.

The Health Center is open 8 a.m. to 7 p.m. Monday through Thursday and 8 a.m. to 4 p.m. Friday during regular academic terms. Clinic hours and physician hours vary. Summer term hours vary and are posted. After hours, students may receive assistance from the Public Safety Office in the Health and Safety Center 208.

All visits to Student Health Services are confidential and a student’s health records are not released without the student’s permission. See this website for more information: http://healthservices.colstate.edu/

History/Constitution Test

A 1975 act of the Georgia Legislature established the requirements that any person receiving a degree from a state-supported institution of higher learning must successfully complete coursework and/or pass examinations in the histories of the United States and Georgia and the constitutions of the United States and Georgia. The act stipulates four requirements: (1) U.S. History, (2) Georgia History, (3) U.S. Constitution, and (4) Georgia Constitution.

Columbus State University students automatically satisfy these requirements if they pass HIST 2111 or HIST 2112 (Survey of U.S. History) and POLS 1101 (American Government) at Columbus State University or the equivalent at a University System of Georgia institution.

Transfer students who have taken their history and/or American government courses outside the University System of Georgia may be deficient in one or more requirements. Those who are determined by the Office of the Registrar not to have met the legislative requirements need to demonstrate knowledge through proficiency tests. These tests do not carry credit hours and do not exempt a student from any core curriculum requirements.

Students who receive credit by CLEP or AP for HIST 2110, HIST 2112, or POLS 1101 are deficient in two of the legislative requirements and must take the proficiency tests.

Information regarding the profeiciency tests, including study materials, may be obtained by contacting the Department of History at 706-568-2263.

HOPE Scholarships

Begun in 1993, the HOPE (Helping Outstanding Pupils Educationally) Scholarship program assists Georgia residents with tuition, fees and books. To be eligible for HOPE, students must be enrolled in degree programs at any campus of the University System of Georgia, at a branch of the Georgia Department of Technical and Adult Education, or at an eligible private college or university in Georgia.

There is no minimum number of hours per semester of enrollment required for eligibility at a public institution. Eligibility for the scholarship is reviewed after every 30 hours attempted. A minimum cumulative GPA of 3.0 is required.

The HOPE scholarship at a public institution covers actual tuition charges and mandatory fees. Students are also eligible for a book allowance of $150 per semester if they are enrolled for 6 or more hours and $75 per semester if they are enrolled for less than 6 hours.

If students have not maintained a cumulative GPA of 3.0 after 30 attempted semester hours, they will be responsible for their sophomore year at their own expense. If they then earn a cumulative 3.0 grade point average after 60 or 90 attempted semester hours, they will be given another chance to receive the HOPE scholarship.

If a student graduated from high school prior to the establishment of the HOPE program in 1993, or if a student is not academically eligible for the HOPE scholarship immediately after high school graduation, the student will, after meeting residency requirements, be eligible for a HOPE scholarship at a public institution after 30, 60, or 90 semester hours attempted with a cumulative 3.0 GPA.

In order to be eligible for the HOPE Scholarship, students must be making satisfactory academic progress as determined by the institution.

Hope will pay for a maximum of 127 attempted semester hours of degree-credit coursework. If a student drops a class during the drop/add period, the course-hours are not counted as attempted. If a student earns a W, WF, and any other letter grade in a course, the course-hours are counted as attempted. Advisors should remind students of the consequence of dropping courses after the drop/add period: the course-hours will count as attempted hours as far as HOPE is concerned and may result in a student having to pay for the last year of college.

Learning support coursework and repeated coursework as well as the grades earned, are counted in the HOPE attempted hours and grade point average. Therefore, the CSU grade point average can differ from the HOPE scholarship grade point average.
